A second methodological concern is the high proportion of observational and indirect evidence relative to direct interventional data. Only Lithgow 2026 is an RCT, and its endpoints were mechanistic rather than clinical. This design imbalance means the synthesis is vulnerable to confounding, selection bias, and reverse causation—limitations inherent to observational vascular research that cannot be resolved by statistical adjustment alone. The over-reliance on observational evidence constrains the strength of causal inference that can be drawn from this synthesis.
